Change To Academic Program Form

This form can be filled out by the student or their advisor in order to change or add to the student's undergraduate major, minor, track, specialization, or delivery mode (online to on-campus or vice versa).

To change a student's graduate degree program (MFA, MS, MBA, MA, PhD, EdD), then need to reapply to MIU here: https://apply.miu.edu/apply/
