Certificate of Approval for Doctoral Dissertation INTAKE Form

IMPORTANT: This form is for use by all doctoral programs at MIU EXCEPT the PhD in Management in China program.

This "Certificate of Approval for Doctoral Dissertation" INTAKE form should be filled out and submitted by the doctoral student themselves. Submission of this form initiates the Certificate of Approval for Doctoral Dissertation completion process.

After submission, all affected parties will be emailed the official "Certificate of Approval for Doctoral Dissertation" form by the Registrar's office.

Each affected party will be asked to fill in certain form fields and/or submit their e-signatures.

When all form information and e-signatures have been collected, each party, including the Library Director (for final approval of the student's dissertation in ProQuest) will be emailed a fully signed and completed copy of the form for their information and records.
