Legal and/or Preferred Name Change Request Form

This form should be filled out by a student who wants their name changed in their MIU student record.

In order to record a legal name change via the form below, proof of your new, legal name is required:

DOMESTIC STUDENTS: You must provide an image of your social security card showing showing your new, legal name.

INTERNATIONAL STUDENTS: You must provide either an image of your a) passport page or b) I-20 record, showing your new, legal name.

Without submitting the appropriate document, we will be unable to proceed with an official name change. However, we can record this (or another) name as your preferred name if you choose.

Please note that your preferred name will not appear on official records like your transcript and diploma; it will only appear on class rosters and your student portal account.
