Degree Audit, i.e. Degree Check, Request Form

This form should be filled out by a current student, their advisor, or their program director in order to request a degree audit, i.e. degree check. A separate/new form submission is required for each request. Unless marked urgent, degree auditors will process requests in the order in which they are received.

Note that we do not provide audits for all programs. If your program is not listed on this form, please contact your advisor or program director for a degree audit.
