Rotating University Program Application INTAKE Form

IMPORTANT: This form is for use by any student who would like to participate in the Rotating University Program at MIU.

After submission, all affected parties will be emailed the official "Rotating University Program Application" form by the Registrar's office. Each party will be asked to fill in certain form fields and/or submit their e-signatures.

When all form information and e-signatures have been collected, the Registrar's office will review the application. If approved, the course will be added to the student's schedule. If not, or if clarification or more information is needed, the student, their academic advisor, and/or instructor will be notified. Once processing has completed, each affected party, including the student, will be emailed a final, completed copy of the form for their information and records.
